Return the completed form in person to the Montgomery County Clerk’s Office, Room 203 of the courthouse OR mail it to Montgomery County Clerk, Room 23, 100 E Main St, Crawfordsville, IN 47933. The office is open Monday through Friday from 8:00 am until 4:30 pm.

You can also apply in person at any BMV license branch if you are obtaining a driver’s license, permit, or identification card.
